Complaint:Failed to protect a resident from mistreatment. Failed to notify the appropriate physician for a traumatic removal and susequent replacement of a peg tube for one of the three sampled residents with peg tubes. Failed to notify the appropriate physician for one of the three sampled residents with weight gain.
