Please use the offender’s name, D.O.C. number and facility if known. On the relationship line, please note your association to the offender. Use the comment lines to provide the Board information not already included in the offender’s file. The Board will have information available on institutional adjustment and programs; therefore you will not need to cover these areas unless you have additional information.
Please list specific ways in which you can provide assistance to the offender in the community if parole is granted (i.e. residence, job, or transportation).
Discuss changes you have observed in the offender.
Note any of the offender’s potential risk areas or concerns.
If you are writing in support of a parole letters may include information regarding employment or potential employment, residence, transportation, available treatment programs (as applicable), you should explain what the offender has accomplished, training opportunities, education plans, or other information the writer feels would be helpful to the Parole Board in making their decision. Include information that demonstrates to the Parole Board that an offender has a support system in place upon release.
The Online Public Comment Form (coming soon) may be used to electronically send your comments to the Parole Board or you may choose to send your comments in writing to the Board Administrative Offices. You will need to have the offender's name, D.O.C. number and docket month and year written on the top of the letter and the back of the envelope. All correspondence to the Board is required to be submitted by 5:00 p.m. on the Tuesday 2-weeks prior to the Board Meeting.
