OPEN RECORDS REQUESTS / REQUESTS FOR BOARD ORDERS

If you would like to request a copy of Board Orders for a licensee that has disciplinary action(s) or if you have any other questions regarding this information, please fill out an open records request form and email your request to the Deputy Director, Jeff Puckett at jeff.puckett@dentistry.ok.gov.

*The lists above only contain active license/permit holder information only. For information regarding an inactive license or permit holder, you will be required to submit a verification request.

NOTE: License verifications are NOT available by telephone.

If you cannot use the above links for verification, or if you are looking for an inactive licensee or permit holder that is not on this list, you must mail in the Verification Request form with a $10 check or money order to obtain a verification.

VERIFICATION OF LICENSURE REQUEST/LETTER OF GOOD STANDING

If you would like to obtain a Verification of Licensure from us, please complete Verification Request form. You will need to print this form and mail it with a $10 check or money order. You may request we send it directly to the state you are applying in, or you can request we send it back to you. Please ensure to read the instructions for the state you are applying for, so you know how they want you to provide that information.
