Hygiene Application by Exam- This is the application method for Dental Hygienists that have taken a Regional Exam within the last 5 years. 

The following exam components are required for a Hygienist:

• Clinical patient treatment with an evaluation of specific clinical skills as well as the candidate's compliance with professional standards during the treatment and shall include:
  • Extra/Intra Oral Assessment
  • Periodontal probing
  • Scaling/subgingival calculus removal and supragingival deposit removal

Hygiene Application by Credential - This is the application method for Hygienists who have taken a Regional Exam more than 5 years ago, who are licensed in another state and have been practicing 5 years immediately prior to making application.

Advanced Procedure Application - Therapeutic Use of Laser

Laser Advanced Procedures Application

Fees are listed on the top of the application and are non-refundable.

1. All applications for dental hygiene licenses must be approved by the Board.  Applications must be received 30 days prior to a meeting in order to be placed on the next regularly scheduled Board agenda.  For Board Meeting dates, please refer to the Reports/Agendas tab of our website.  

2. After your application has been approved by the Board, you will be contacted via email to schedule a date/time to take the Jurisprudence Exam.  You may begin studying for the Jurisprudence Exam immediately by clicking on the Statutes and Rules tab.

3. Exams are only given on Thursday's by appointment only and are only proctored at the Board office.
