HealthChoice Launches Phase Two of Wellness Initiative

HealthChoice is excited to announce the launch of the second phase of its wellness initiative.  Phase two, called HELPCheck Logo (Help Check), begins July 1, 2011, and is available to all primary HealthChoice health plan members who are age 20 and older and not on a Medicare supplement plan.  HELPCheck Logo combines the results of certain free preventive services with an online, interactive health risk assessment (HRA) which is designed to provide members with personalized health information, as well as the steps they need to take to actively improve their health. While the program is open to all health plan members as described above, primary plan members who complete the requirements of HELPCheck Logo receive a $100 incentive payment from the Plan.

To qualify for the incentive payment, eligible members must complete the following preventive services:

  • One comprehensive metabolic panel
  • One comprehensive lipid panel
  • One comprehensive preventive medicine visit

Please refer to the article New Coverage for Preventive Services in 20ll for the specific CPT codes that allow these services to be covered at 100% of the HealthChoice Allowed Charges with no cost to the member. These services do not have to be provided on the same date; however, they must be provided in an outpatient setting and billed using the appropriate codes for the member to receive the 100% benefit.

Once the member completes their preventive services, they will need your office to provide certain information and test results that are required to complete their HRA. HealthChoice has developed a special form members can use as a tool for collecting their health information. The form provides a place to record the following required measurements:

  • Height/Weight
  • Blood Pressure
  • Tobacco Use
  • Fasting Glucose
  • LDL, HDL, and Total Cholesterol
  • Triglycerides

After members complete their HRA, and include the information listed above, HealthChoice will issue qualified members a check for $100. Regardless of whether they earn a check for their participation, HealthChoice hopes members will be motivated to adopt a healthier lifestyle.
