HELP Check web header Program Information

Exam ToolsHealthChoice is continuing its wellness initiative, called HELP Check Logo (Help Check) for Plan Year 2012. HELP Check Logo is available to all primary HealthChoice health plan members age 20 and older who are currently working or are retired but not eligible for Medicare. 

HELP Check Logocombines the results of certain free preventive services* with an online, interactive health risk assessment (HRA) that is designed to provide members with personalized health information, as well as the steps they need to take to actively improve their health. Primary members who complete the requirements of HELP Check Logo receive a $200 incentive payment.

 

Let's Get Started!

  1. Read your HELP Check Logo Instruction Packet
  2. Have your free preventive services office visit* (see the Instruction Packet for the Provider Billing Instructions)
  3. Have your free, fasting blood tests*
  4. Complete the health risk assessment (HRA)**
    1. Go online to the HRA
    2. Answer the health questions
    3. Enter the following required information:
      • Height and Weight
      • Resting Heart Rate (Pulse)
      • Blood Pressure 
      • Fasting Blood Glucose
      • Fasting Cholesterol (HDL, LDL, Total Cholesterol, Triglycerides)
      • Answers to the questions: 
        • Do you smoke?
        • Do you use tobacco products?
    1. Review your HRA results

Make the commitment to participate in the program, take steps toward a longer, healthier life, and earn $200.

You must complete your preventive services visit, comprehensive metabolic and lipid panels, and your HRA, including your lab results, by December 31, 2012, to qualify for the $200 incentive payment for Plan Year 2012.

*These services are covered at 100% of Allowed Charges with no copay or out-of-pocket costs to you when provided in an outpatient or medical setting by a HealthChoice Network Provider. For these services to be covered at 100%, specific CPT codes must be used when billing.

**If you have never accessed the health risk assessment (HRA), it is not available to you at this time. HP Administrative Services, LLC (HP) is working to correct the issue. We apologize for the inconvenience.
