Requests for Claims Files and Open Records

Individuals who seek records of prior claims related to work injuries or open records may complete and submit the appropriate form below based on the type of record sought. Please note that certain fees may apply based on our Schedule of Fees.

Prior Claims Files

(date of injury on or after February 1, 2014)*

Pursuant to 85A O.S. § 120, all requests for claims file information pertaining to work injuries should be made using the Request for Prior Claim File Information form. Completed forms or questions about this form may be emailed to Records@wcc.ok.gov.

*Note: Records related to claims of an injury occurring before February 1, 2014, should be directed to the Oklahoma Workers’ Compensation Court of Existing Claims.

Open Records

The purpose of the Oklahoma Open Records Act is “to ensure and facilitate the public’s right of access to and review of government records so they may efficiently and intelligently exercise their inherent political power.” 51 O.S. § 24A.2.

Individuals wishing to submit an Open Records Act request should fill out the  Open Records Request form.
