Inadequate Staffing Report

Applications & Renewals:

 

  • NEW Technician Application
    • See reinstatement application if permit has been canceled.
    • Permit will expire annually on the last day of birth month.
  • 2022-2023 Pharmacy Technician Permit Renewal [paper renewal for permits expiring in the year 2022]
  • 2021-2022 Pharmacy Technician Permit Renewal [paper renewal for permits expiring in the year 2021]
  • Technician Permit REINSTATEMENT Application 
    • Canceled technicians cannot apply for a new permit.  Once a permit has been issued and then canceled (for whatever reason), you must reinstate that permit upon returning to work as a technician.
    • A technician permit reinstatement cannot be completed using online renewal.  A paper reinstatement application must be completed and submitted to the Board.
    • Unless you are currently employed in a licensed Oklahoma pharmacy and under the immediate and direct supervision of a licensed Oklahoma pharmacist, the Board will not reinstate your technician permit.
    • Permit will expire annually on the last day of birth month.

Duplicate Permit Request for Multiple Employment:

  • Technician Multiple Employment Report Form & Duplicate Permit Request 
    Technicians who did not renew online during their last renewal or new technicians who have not yet renewed their permit must submit this form upon multiple employment (i.e. employment at more than one pharmacy location).  Technicians who have previously renewed their permit online may log-in to the
    OSBP License Renewal page and print a duplicate permit at any time.  

Duplicate Permit Request for Change of Employment or Lost Permit:

Technicians who did not renew online during their last renewal or new technicians who have not yet renewed their permit must submit a written request to the Board by mail (no form is required) for a duplicate permit along with a $10 fee.  The request should state the reason for the duplicate request such as a lost permit or a change of employment, name and permit number.  Technicians who renewed their permit online during their last renewal may log-in to the OSBP License Renewal page and print a duplicate permit at any time.

Announcements

Technician permits must be signed by a licensed Oklahoma pharmacist to be valid. Unless you are currently employed in a licensed Oklahoma pharmacy and under the immediate & direct supervision of licensed Oklahoma pharmacist who will sign your permit, your permit will not be valid.

FAQs

Q: How old do you have to be to apply for a pharmacy technician permit?
A: Technician applicants MUST have either a High School Diploma or a GED before they can apply for a permit.
Q: I have a tax hold on my permit. What does that mean?
A: The Oklahoma Tax Commission places a "tax hold" on technicians who are not in compliance with state income tax laws. The Board cannot process any application from a registrant that has a hold. Call 405-522-6800 to get your hold released.

Interesting Facts

State law requires that changes in mailing address or employment must be reported to the Board of Pharmacy within 10 days [see 535:15-13-10]. Reports must be submitted in writing by mail, e-mail or fax.
Pharmacist & Technician renewal reminder postcards are mailed 60 days in advance of expiration. If needed, registrants may download a renewal application from the Board website.
