IMPORTANT: BEGINNING JULY 1, 2021 OSBP WILL NO LONGER ACCEPT CASH, MONEY ORDERS OR CHECKS AS A FORM OF PAYMENT.  IF RECEIVED, THE PAYMENT WILL BE RETURNED TO THE APPLICANT. ALL PAYMENTS SHOULD BE MADE THROUGH OUR ONLINE STORE.

Inadequate Staffing Report

Renewal Applications (paper form):

  • 2022-2023 Notice of Renewal of Pharmacist Licensure [paper renewal for licenses expiring in the year 2022]
  • 2021-2022 Notice of Renewal of Pharmacist Licensure [paper renewal for licenses expiring in the year 2021]

Additional specialized permits:

  • Preceptor Certificate Application - Only Pharmacists who have been licensed for a minimum of 1 year (in the United States) are eligible for a Preceptor License
  • Immunization Registration Application - Interns and Technicians are NOT eligible for Immunization Registrations- This is for Oklahoma licensed pharmacists ONLY.  This is for initial registration only, if you need a duplicate copy of an existing registration, please submit a written request with a $10 payment.

Duplicate certificates:

  • Application for Duplicate Pharmacist Certificate   [if lost, destroyed or name change]
    • The above application is for the "big" wall certificate. 
    • Lost "renewal cards" can be replaced by sending a written request for a duplicate renewal card and a $10 fee to the Board office or if you completed your annual renewal online, you may log-in to the OSBP License Renewal page and print another renewal receipt.
  • Application for Doctor of Pharmacy  (D.Ph.) Certificate
    • Optional for pharmacists who currently have a Registered Pharmacist (R.Ph.) certificate

FAQs

Q: I'm a new graduate who has recently taken the NAPLEX and MPJE exam. When can I begin working as a pharmacist?
A: You cannot practice pharmacy in Oklahoma until you have received an official letter of registration from the Board. You should receive your scores and registration letter within 15 WORKING DAYS from the date of your last exam. Website verification is not an official certificate of registration.
Q: I have a tax hold on my license. What does that mean?
A: The Oklahoma Tax Commission places a "tax hold" on pharmacists who are not in compliance with state income tax laws. The Board cannot process any application from a registrant that has a hold. Call 405-522-6800 to get your hold released.

Interesting Facts

Pharmacist & Technician renewal reminder postcards are mailed 60 days in advance of expiration. If needed, registrants may download a renewal application from the Board website.
