Inadequate Staffing Report

 

Renewal Applications:

  • 2023-2024 Notice of Renewal for Pharmacy License [for licenses expiring in the year 2023]
  • 2022-2023 Notice of Renewal for Pharmacy License [for licenses expiring in the year 2022]

Pharmacy License Application (new or change)

  • Pharmacy License Application [also see ownership forms below]

    The Pharmacy License Application must also have one of the following ownership forms attached.  Please choose your type of ownership and attach the appropriate form to your application:
    • Form A - Sole Proprietor or Partnership
    • Form B-1 - Corporation (not publicly traded)
    • Form B-2 - Corporation (publicly traded)
    • Form C - Limited Liability Company
    • Form D - Government

Pharmacy licenses are non-transferable! A new application is required for any change of owner, change of location or change of name.

Announcements

Effective November 1, 2015 all pharmacy license and pharmacy license renewal applications will be required to have an Oklahoma licensed pharmacist in the position of Pharmacist-in-Charge. (see notice below)
Effective August 27, 2015 pharmacies may no longer compound non-patient specific sterile drugs for sale to physician offices or hospitals to administer. (see Memo below)

FAQs

Q: When will I receive a renewal for my license?
A: Renewal applications for pharmacies and facilities are mailed approximately 60 days in advance of the date of expiration. If needed, registrants may download a renewal application form from the Board website.

Interesting Facts

Prior to administering immunizations, pharmacists must complete a Board approved immunization training course and register with the Board.
