To ensure the safety and protection of the public, registrants, and OSBP staff, the Oklahoma State Board of Pharmacy Office is currently closed to the public. Staff members are in the office conducting business, and able to assist you with any questions. OSBP staff is confident all of your questions can be addressed through phone or emailed requests. Documents can be hand-delivered to a drop box at the office, but because it is a "shared" box, documents should be in an envelope and clearly addressed to OSBP.
IMPORTANT: BEGINNING JULY 1, 2019 OSBP WILL NO LONGER ACCEPT CASH AS PAYMENT. ANY CASH RECEIVED WILL BE RETURNED TO APPLICANT.
Each new application or application for change of Name, Ownership or Location must also have one of the following ownership forms attached. Please choose your type of ownership and attach the appropriate form to your application:
Licenses are non-transferable! A new application is required for any change of owner, change of location or change of name.
