PDH Log Form
Each licensee is charged with the responsibility of his/her own professional development activities. The responsibility of maintaining records to be used to support credits claimed is the responsibility of the licensee.
The licensee should keep a log that shows the date, sponsoring organization, activity title, location (city and state) and continuing education credits earned.
