Submitting Your Application Form D - Part Two
After completing an approved education program, passing both the FE and PE exams, and obtaining your required experience you will submit Application Form D – Part Two to complete the licensure process.
Do NOT submit an Application Form D - Part Two until you have been notified that you have passed the P.E. or S.E. exam AND you have obtained the full amount of progressive engineering experience required for licensure based on your education.
- Submit either the Application Form D - Part Two for NCEES Record Holders OR the Application Form D - Part Two Application for Non-NCEES Record Holders (Do Not Submit Both).

Application Form D - Part Two for NCEES RECORD HOLDERS
An NCEES Record will streamline the multistate licensure application process for you in the future. If you plan to practice engineering in multiple states, consider applying for initial P.E. licensure in Oklahoma through the NCEES Records program. Information regarding obtaining a NCEES Record can be found at https://ncees.org/records/. Please follow the instructions and contact the NCEES Records Department at 800-250-3196 if you encounter any problems. You must request that NCEES transmit your NCEES Record to us; we cannot do this for you. NCEES will charge a fee of $100 to transmit the completed NCEES Record to us. You will then have a NCEES Record that you can maintain throughout your career with no charge for updates and a reduced fee of $75 for each transmission to other states where you seek licensure.

Use this application if you will NOT be requesting that NCEES transmit a NCEES Record to the Oklahoma Board as part of your initial licensure application.
