P.E. and P.E., S.E. Initial Licensure Applicants - Form C
For applicants who have passed the P.E. examination and completed all experience requirements.
Application for Early Admission to the PE Exam (Decoupling): Form D - Parts One and Two
If you have completed your qualifying degree requirement and passed the Fundamentals of Engineering Exam (FE Exam) you may take the P.E. Exam at any time you feel you have sufficient experience to pass the examination.

If you plan to provide engineering services through a firm you must download the Certificate of Authorization Application and submit it along with your P.E. application.
