If the job you wish to apply for is a classified position, you must also submit an OPM Personal Data Summary Sheet (Form 4-B) or a current OPM Form 9 (dated within the previous six months). These forms are available from your facility's Human Resources Management office or the Human Resources Management Division in the ODMHSAS Central Administrative Offices.
