DISTRIBUTION OF NURSE AIDE
CERTIFICATION RENEWAL CARDS

The Oklahoma State Department of Health (OSDH), Nurse Aide Registry (NAR), will no longer print and mail certification cards for initial certification or the renewal of nurse aide certification(s) in Long Term Care (LTC), Home Health Aide (HHA), Developmentally Disabled Direct Care Aide (DDCA), Residential Care Aide (RCA), Adult Day Care Aide (ADCA), and Certified Medication Aide (CMA) (including advanced CMA classifications).
Certified nurse aides will receive a certification card with their initial placement on the Nurse Aide Registry. This is the only certification card which will be mailed to the nurse aide. The card will give the date the aide was first certified and placed on the Nurse Aide Registry, but without an expiration date. It will state on the card, “Verify the status of certification at: http://nar.health.ok.gov “
Certified Nurse Aides can click on "Verify a Nurse Aide is on the Registry" to verify and print certification status. Employers are required to verify certified nurse aides status through the National Background Check Program and can also use "Verify a Nurse Aide is on the Registry" for their records.

Nurse Aide Training And Competency Evaluation Programs
The Nurse Aide Registry (NAR) is contracted by the Centers for Medicare and Medicaid Services to review and approve Nurse Aide Training Programs (NATP). Pursuant to the Code of Federal Regulations (CFR) at Title 42, Section (§) 483.151(b)(1)(iii), in all reviews other than the initial review, the State must visit the entity providing the program. The State may not grant approval of a NATP for a period longer than 2 years as specified in 42 CFR § 483.151(d).

The attached sign-in sheet was offered by Anita Hasenmyer, Red River Technology Center, as a tool they have found useful in documenting the certification numbers of CMA/CEU attendees. The program uses the certification numbers to confirm the certification is current prior to issuing the CMA/CEU. Training Programs may use this form for a sign-in sheet for their students. Please do not mail hand-signed original form to Nurse Aide Registry.
Please mail in a typed form or letter with the information stated in OAC 310:677-13-12(d) as shown below (Title of Class, number of hours, name of student, certification number). Also, please show date of class.
OAC 310:677-13-12(d) An approved continuing education program shall submit to the Department within 30 days after the conclusion of a continuing education class the following information:
(a) the certified medication aide training program shall include a minimum of forty (40) hours of combined classroom and supervised practical training with a minimum of sixteen (16) hours of supervised practical training.
An approved training program may use the CMA Skills Performance Checklist as shown below:

The facility, center or home shall validate certified medication aide skills before the certified medication aide performs medication administration. the certified medication aides; skills shall be reviewed annually for performance competency.

Oklahoma law provides that nurse aide training programs must include ten (10) hours of training in the care of Alzheimer’s patients.
63 O.S. ��? 1-1951(A)(3) The State Department of Health shall have the power and duty to determine curricula and standards for training and competency programs. The Department shall require such training to include a minimum of ten (10) hours of training in the care of Alzheimer’s patients;

The Oklahoma Chapter of the Alzheimer's Association endorses the Barbara Broyles Alzheimer and Dementia Training Program for Nursing Assistants for nurse aide training programs. The linked curriculum may be freely adopted and incorporated into your curriculum. Please provide appropriate credit.

Notice of Change: OAC 310:677-3-5(c) An approved program shall notify the Department in writing before making substantive changes to the program.
Affirmation of Required 16 hours of training: OAC 310:677-11-4(a)(2) At least 16 hours of training in the following areas prior to any direct contact with a resident that is documented and signed by the nurse aide trainee.
