RENEWAL OF LONG TERM CARE CERTIFICATION ONLY

If you only have a long term care certification please renew with ODH Form 840.

 

ODH FORM 840 – Long Term Care Recertification Application

You may renew your certification six (6) months in advance using the following methods:

  1. Mail to OSDH/Nurse Aide Registry, 1000 NE 10th ST, Oklahoma City, OK  73117
  2. E-mail to NAR@health.ok.gov
  3. Fax to (405) 271-1130

You will no longer receive certification cards.  You can print your verification of certification on our web site at “Verify a Nurse Aide is on the Registry”.  Facilities are not to request a card.  They are to verify your certification on our website per regulation.

If you have not worked during you’re the time of your two (2) year certification or your certification has expired by more than two years, you will have to retest to keep your certification.  To retest you will need to send the Nurse Aide Registry.  When we receive it we will mail you an original retest letter.  You can then contact any Career Technology Center to schedule your retest.  You only have one chance to pass the test.  If you do not pass you will have to retrain for your certification. 

You can send in your retest application using the methods above for long term care certification only.

RENEWAL OF ALL OTHER CERTIFICATIONS

If you are certified as a Home Health Aide, Long Term Care Aide, Residential Care Aide, Developmental Direct Care Aide, Adult Day Care Aide, Feeding Assistant, or Certified Medication Aide and many have Advanced Certified Medication training please use ODH FORM 717.
