State Vital Records Office to Stay Open Late Each Thursday in August

To help ease the rush for busy parents who need a copy of their children's birth certificates before enrolling them in school in August, the state Vital Records Office will remain open until 6:30 p.m. each Thursday evening in August.

The office is located on the first floor of the Oklahoma State Department of Health, 1000 N.E. 10th Street, Oklahoma City. Visitor parking is available.

To obtain a copy of a birth certificate, applicants will need to provide a date of birth, place of birth, parents' names, and furnish a photo ID. There is a $5 search fee for each certified copy of a birth certificate.

During the later office hours on August 3, 10, 17, 24 and 31, persons may also obtain copies of needed death certificates. For a death certificate application, applicants must provide the person's name, date and place of death. There is a $10 search fee for each certified copy of a death certificate.
