The Wagoner County Family Service Council, Wagoner County Health Improvement Partnership, and Wagoner County Health Department are partnering to conduct a Community Health Assessment to develop a Community Health Improvement Plan for Wagoner County. There are four assessments that form the Community Health Assessment and we are asking for your participation in completing one of the four. This assessment is the Community Themes and Strengths Assessment and it asks questions based on your perception of the health and quality of life in Wagoner County. This assessment is completely anonymous and will not be traced to an individual. We would like to get as many participants as possible to try and get a good representation of what the public feels of our community. Most programs are offered through our main office in Wagoner, as well as our satellite clinic in Coweta, allowing for convenient access to all areas of our county.
