The mission of the Oklahoma Funeral Board is to act in the public interest, and for the protection of the public within the powers vested in the Board by the State of Oklahoma for the regulation and education of the funeral service profession.
All information provided on this web site is unofficial and only offered as a convenience to the public. All official records and information can be obtained by contacting the Oklahoma Funeral Board Office
The Oklahoma Funeral Board's hours of operation are 8:00 AM to 4:00 PM. If you would like to meet with a staff member, please set up an appointment by email with staff prior to arrival. You may also email staff if you need confirmation from a staff member of receipt of documents. If you wish to deliver documents, our offices are currently located at:

The Oklahoma Funeral Board in an effort to avoid uninterrupted services to the general public will continue to operate and offer assistance to the general public and funeral homes across the State. We have taken measures to ensure that the mission of the Oklahoma Funeral Board is fully executed and to ensure the public’s health, safety and welfare are protected. Currently the office staff is teleworking with intermittent visits to the office. We will do our very best to continue to work at a 100% capacity with our available resources to provide sufficient services to the general public and Oklahoma licensees. The Oklahoma Funeral Board along with other State agencies will continue to work with other state and local government agencies to provide information to all licensees and the general public as we receive information on the Coronavirus (COVID-19). We encourage you as death care providers to continue to communicate with staff on proper sanitary measures as well as communication to the families you see in your Funeral Home. You can access further information at the Oklahoma State Department of Health’s website. In addition to visiting the State Department of Health’s website the Call Center number and hours are being provided to further assist in questions, you or your patients may have regarding Coronavirus (COVID-19).

Click link below to download a blank copy of the 2020 Funeral Director and/or Embalmer Renewal Form. Cost to renew each license is $75.00, so to renew both the cost is $150.00. As a reminder, all renewals must be postmarked by December 31, 2019, otherwise, licensee will incur a late fee.
