||The Office of Management and Enterprise Services/ISD Procurement (OMES), on behalf of the Oklahoma Department of Human Services (DHS) is soliciting offers from suppliers to procure the services of an organization to provide a Commercial Off the Shelf (COTS),web-based information system solution in support of 1915(c) Home and Community-Based Service (HCBS) Waiver programs and complying with all federal and state rules, regulations, laws, statutes, polices and/or procedures including those applicable to specific Centers for Medicare and Medicaid Centers for Medicaid and Medicare Services (CMS) approved 1915(c) HCBS Waivers.

- In order to receive payments from the State of Oklahoma, suppliers who are not registered on the State of Oklahoma Vendor Registration list must complete the “Vendor/Payee Form” Word (3 pp, 160 KB) | PDF (3 pp, 137 KB). Non-U.S. suppliers who are not registered on the State of Oklahoma Vendor Registration List must complete a W-8BEN. Failure to do so may delay contract award.
