License Renewal: Continuing Education Roster

The Continuing Education Roster is available online in two formats. Please read the information below and determine which roster is best for your needs.

The following roster has been formatted with Adobe Pro. This form can be completed on your computer. If you are unable to download this form you may need to download the free Adobe update on the left-hand side of this page. Once completed, you will need to print your roster and submit it to the office of Professional Counselor Licensing, along with your renewal fee.

The following form is an Adobe file and does require you have Adobe installed on your computer. However it is not formatted in Adobe Pro and does not require any additional updates. This form cannot be completed on your computer. Please print, complete and submit your roster to the office of Professional Counselor Licensing, along with your renewal fee.

If you are unable to download either of the above forms, you may request a roster be e-mailed to you in another format sent or by mail. Submit your request to Eric Ashmore at erica@health.ok.gov.
