Inactivate Status/Reactivation and Retirement:
You may place your license on inactive status, if your license is in good standing, by submitting a request to be placed on inactive status, a one-time $25.00 fee, and your original license. You do not need to accrue continuing education or pay any additional fees while your license is inactive.
You may reactive your license by submitting a reactivation request form and the prorated fee for the month you wish to reactive. Additional information regarding reactivation requirements is on the reactivation request form.

If your license is in good standing, you may retire your license by notifying the Department in writing. Although a retired license cannot be reactivated, it does not prevent an individual from reapplying for licensure in the future.
