Application Instructions

MAIL OR HAND DELIVER 3 copies of the completed application and supporting materials to the Oklahoma Arts Council (address below) either postmarked December 15, 2007 or hand delivered by 4:30pm on that day.

Application should be typed or filled out on a computer using the latest version of the free software Acrobat Reader.
Support materials that must accompany this application include:
- 1 Cover Letter
- Curriculum Vitae
- Photo (minimum 3 x 5 print)
- Sample lesson plan and rubric
- Body of work: Do not submit original artwork or manuscripts
  - Visual Artists- portfolio, max. of 10 images on CD
  - Performing Artists- Video 5-10 minutes DVD (no VHS) of a recent performance no more than one year old representative of the programming submitted. (Musicians audio CDs – 3-5 minute segment, note selected track(s).
  - Media Artists (film, video, digital arts) - Video DVD (no VHS) and stills/ samples of short films, videos, storyboards, etc., list of technical equipment owned for residencies.
  - Creative Writing- portfolio, maximum of 15 pages of writing in poetry, drama, and/or fiction (short stories or novel excerpts) representative of programming to be used during residencies.

NOTE: All support materials submitted with the application will become property of the Oklahoma Arts Council. No support materials will be returned. Do NOT submit original/irreplaceable materials. Adobe Acrobat required.
