for SEED OK Participants

If Study Participants meet certain eligibility requirements, savings in SEED OK will be matched up to $250 per year. The Match Eligibility Form allows the state to check records and see if Study Participants are eligible for the Savings Match. This Form is valid until the SEED OK study ends. Along with the Match Eligibility Form, Study Participants who did not file a 2006 Oklahoma State Income Tax Return and did not receive certain benefits from the Oklahoma Department of Human Services during 2006 should complete the Self-Certification Form to determine eligibility for the Savings Match.
