HealthChoice Coverage for Depo Provera

As a reminder, the injectable, Depo-Provera, and its generic equivalents were moved from coverage under Pharmacy to the Health Plan for charges incurred on or after January 1, 2012. Certain members, who were previously using this medication, can obtain the medication at the pharmacy through the end of the 2012 plan year.

Effective January 1, 2013, all HealthChoice members using injectable forms of Depo-Provera or the generic equivalents must obtain this medication through their health provider.
