CPT Category II Codes (F Codes)

Effective for charges incurred on or after January 1, 2012, HealthChoice and the Department of Corrections (DOC) will recognize Category II Codes (F Codes) only for reporting purposes. Procedures and/or services reported using these codes are not covered and are not eligible for reimbursement.
