Providing Information in a Way That Works For You

HealthChoice is required to provide you information about plan benefits that is accessible and appropriate for you. If you have trouble getting information about your plan because of problems related to language or disability, HealthChoice will work with you to provide plan materials in an appropriate format. If HealthChoice does not respond appropriately to your request, you can file a complaint with Medicare by calling toll-free 1-800-MEDICARE (1-800-633-4427), 24 hour a day, 7 days a week. TTY users call toll-free 1-877-486-2048.
