HealthChoice Medicare Supplement Plans With Part D 
Grievance and Appeals Summary for Pharmacy Benefits

Federal law guarantees your right, as a plan member, to make complaints if you have concerns or problems with care or services. The Medicare program has helped set the rules about what you need to do to make a complaint, and what HealthChoice is required to do when a complaint is made. If you make a complaint, HealthChoice must be fair in how it is handled. You cannot be disenrolled from this Plan or penalized in any way for making a complaint.
