Fosamax D Benefit Change for HealthChoice Pre-Medicare Members

Beginning April 1, 2012, Fosamax D is no longer covered without a prior authorization for pre-Medicare HealthChoice members. You must first try the Preferred generic, alendronate, before you can obtain a prior authorization for access to Fosamax D.
