HealthChoice Announces Online Health Risk Assessment

HealthChoice recently implemented an online health risk assessment (HRA) for its health plan members. The HELPCheck Logo HRA is a secure, HIPAA-compliant system designed to:

  • Provide immediate, personalized health information
  • Increase awareness of personal health risks
  • Encourage active participation in health improvement
  • Provide online education about lifestyle changes and other steps that can be taken to eliminate and/or reduce health risks and improve overall health

To access the HRA, click the HRA button on the homepage, log into ClaimLink, and then click the Health Risk Assessment tab in the top menu bar. It takes about 15–20 to take the HRA.

If you are not already registered for ClaimLink, you will be prompted to register and select a unique user ID and password.

To get immediate, personalized health information and video counseling, HealthChoice encourages you to complete the HRA online. If you don’t have Internet access at home or at work, your local library has computers with Internet access and many businesses now have free Wi-Fi for customers. If none of these options are available to you, you can contact HP Administrative Services at 1-405-416-1800 or toll-free 1-800-782-5218. A representative will contact you to discuss options for completing the HRA online. TDD users can call 1-405-416-1525 or toll-free 1-800-941-2160.

As a last resort, a paper HRA will be mailed to you to complete and return.
