Retiree Option Period Meeting Schedule
Plan Year January 1 through December 31, 2013
The Oklahoma State and Education Employees Group Insurance Board (OSEEGIB), a division of the Office of Management and Enterprise Services, has scheduled meetings across Oklahoma to assist you with the annual Option Period enrollment process. There are two types of meetings scheduled:
Medicare Eligible Members
Medicare Supplement Plan meetings are set for 10:00 a.m. OSEEGIB staff will present information on the available Medicare Supplement Plans. Representatives from the Medicare Advantage Prescription Drug (MA-PD) Plans will attend the meetings held within their service areas. If you will become eligible for Medicare on or before January 1, 2013, you should plan to attend one of the meetings for Medicare eligible members.
Pre-Medicare Members
Pre-Medicare meetings are scheduled for members who are under age 65 and not currently eligible for Medicare. All of these meetings are set to begin at 1:30 p.m.
Meetings begin on October 15, 2012, and run through November 28, 2012.

ATTENTION PRE-MEDICARE FORMER EMPLOYEES, COBRA PARTICIPANTS, AND SURVIVING DEPENDENTS
Current and future pre-Medicare HealthChoice health plan members, your action is required! If you are a non-tobacco user, you will want to complete the tobacco-free Attestation form included with your Option Period Enrollment/Change Form or complete the tobacco-free Attestation available online. Members who do not or cannot attest to being tobacco free will have a higher deductible and out-of-pocket limit for Plan Year 2013. Be sure to read your Option Period guide when you receive it and attend one of these important meetings for more information.

Your Option Period form must be postmarked no later than December 7, 2012. |
*Medicare Advantage Prescription Drug (MA-PD) Plan representatives will attend.
Reminders
Option Period
Your Option Period Enrollment/Change Form and Option Period guide will be mailed to you prior to the meetings. To protect your personal information, forms are mailed in a security envelope separate from the Option Period guide. Please bring your Option Period materials with you when you attend one of these meetings. Option Period forms, including your tobacco-free Attestation form, must be postmarked no later than December 7, 2012.
Moving
If you move, you must notify OSEEGIB in writing of your new address or download a Change of Address Form. Please mail your change of address information to:
