Oklahoma Identity Theft Passport Program

OSBI Responsibilities
1. Make application available to applicants (in person, mail, fax, or OSBI web site.
2. Provide Instructions with application. Instructions will clarify required documents
3. Required documents will include court orders of expungement for first group of qualified applicants, and copy of police report indicating the report of the applicant being a victim of identity theft; a copy of the identity theft affidavit found on the Federal Trade Commission web site at www.ftc.gov; a copy of the certified mail delivery receipt showing the affidavit received by one of the three major credit reporting bureaus; 2 color, passport style photographs of the applicant to be used for the file and the passport; a copy of the applicant’s Oklahoma drivers license or identification card (or military identification and other state drivers license if Oklahoma DL or ID Card is not available) to be used for verification of applicant’s identity; if the applicant is a minor, then the application must contain the photographs of the minor and a copy of the minor’s parent or legal guardians’ Oklahoma drivers license or identification card, with a sworn and notarized affidavit attesting to the fact that the subject is the applicant’s parent or legal guardian.
4. Confirm all required documentation has been supplied.
5. Issue the Passport Card and Certificate by certified mail.
6. Notify CHMU of registration so CCH and SDA files can be flagged.
7. Notify DPS of registration so DPS/OLETS can be flagged. (Formal contact to be identified by DPS at later date, until then, notification will be by mail to Mr. Dave Maloy, Interim Director, OLETS, Oklahoma Department of Public Safety.
8. File application and contents, maintaining confidentiality of contents except to law enforcement.
9. Other than status of passport registration, investigations into the contents of the application file will require a written request from the agency head of the law enforcement agency making inquiry, with a statement that the inquiry is for law enforcement purposes only.
