A replacement will be returned to you within 14 days. Q: How do I change my name on my license? A: Send a notarized letter requesting the name change along with legal documentation showing your current name (ie Marriage license, Divorce Decree) and a $15 Money Order or Cashier's Check (for replacement license) to the OSBI, SDA unit. Q: How do I change my address on my license? A: Mail a notarized letter indicating the change of address to the OSBI, SDA Unit. Both the old and new address must be included in the letter. If a replacement license is desired, include a Money Order or Cashier's Check in the amount of $15. Q: Which Conceal Carry permits does the State of Oklahoma honor? A: Per Title 21 § 1290.26, the State of Oklahoma recognizes any valid concealed carry weapons permit or license issued by another state.
