Simplified (and generalized) steps 
for licensure application

 

The steps are slightly different for each type because requirements, training and qualifications vary for each type. 

If you have not reviewed the qualifications for the type of license you wish to apply for, you should FIRST do that, in all cases...if you have an adverse reaction to a requirement for reading, you may well be entering the wrong profession as this is one of the most regulated "industries" known... but we set the only link to THIS page on our website from that page (qualifications) so theoretically, you should have been exposed to them (unless you skipped ahead to the link like too many people in a hurry are wont to do...).


 

STEPS FOR NURSING HOME ADMINISTRATOR (NHA) and ADMINISTRATOR UNIVERSITY (AU) APPLICANTS

NOTE:  There will be slight variations for those applying for licensure by endorsement, for a provisional license, for a temporary license, and to be a Certified Assistant Administrator

- Have a "passport" type photo (head and shoulders view) ready to upload when you apply...

- Go to our Forms and Fees page and complete the form(s) you need there online; PRINT IT and have it notarized; then scanned into a PDF and ready to upload..

- Enroll in our online application system - (follow this link) ; you will be using your email address and assigning your self a password

- Read and follow the instructions you receive in an email you will receive after you enroll.  

- Log in to our system through our online application portal  (your email address is your USERNAME...and only you know the password you assigned yourself when you enrolled).

- Complete the application questions and upload the required documents..., a passport type photo (head and shoulders view), the notarized form (Citizenship)... documentation regarding any other explanations to other questions that may require such an explanation; your resume; three reference letters.  Do NOT upload your transcript(s) as those must come to us from your college/university; those applying for licensure by endorsement, do NOT upload those questionnaires either - they should come from the licensure boards in those other states.

  -- AU students need to apply for AU and will also be asked to complete a self-assessment questionnaire as part of the process

  -- Those applying for Licensure by Endorsement need to provide that questionnaire to the other state(s) licensing board(s) and have them mail them to us directly

  -- All except Certified Assistant Administrator applicants need to have their college/university transcripts sent directly to us (do not upload those yourself)
   -- Certified Assistant Administrator applicants can have their transcripts or high school diploma or GED send directly to us (do not upload those yourself)

  -- All applicable fees must be paid by all applicants (AU has a fee, there's a fee to apply, AIT will have a fee when that time comes...non-AU applicants will also have to pay for the State Standards Exam and may get a study packet mailed to them for the State Standards exam for a fee)

- Complete your applicable training (NHA applicants - normally this means AU and AIT...others may have no training requirements)

- Take and pass the applicable State Standards Exam (NHA) 

  -- The State Standards Exams are scheduled periodically (coordinated with AU and other times) and non-AU applicants will need to "Register" and pay for the exam through our online "calendar" which is linked at the top of every page on our website.  There is a fee for the Exam  ($100) and a fee for licensure ($200)

  -- Passing the NHA State Standards exam may complete the requirements for certain applicants (Provisional, Endorsement, Certified Assistant) and only needs to be approved the Board at its next meeting (you should attend if able).

  -- If the NAB NHA exam is required (most AU students...a few may have already passed it),  and have completed all training (AIT), the applicant will be eligible to take the NAB NHA exam

     --- Once the RC/AL applicant has passed both the State and NAB exams, he is eligible to be licensed at the Board's next meeting.

     --- If an NHA applicant has met all requirements for licensure, they MAY apply for a Temporary License (which expires at the next Board meeting) and the Executive Director may approve it, enabling them to work as an NHA sooner (as explained in the qualifications page)


 

STEPS FOR RESIDENTIAL CARE (RC) and RESIDENTIAL CARE/ASSISTED LIVING (RC/AL) APPLICANTS

- Complete your training with an approved source and have your training completion certificates scanned into a PDF format ready to upload when you apply...

- Have a "passport" type photo (head and shoulders view) ready to upload when you apply...

- Go to our Forms and Fees page and complete the form you need there online; PRINT IT and have it notarized; then scanned into a PDF and ready to upload..

- Enroll in our online application system - (follow this link) ; you will be using your email address and assigning your self a password

- Read and follow the instructions you receive in an email you will receive after you enroll.  

- Log in to our system through our online application portal  (your email address is your USERNAME...and only you know the password you assigned yourself when you enrolled).

- Complete the application questions and upload the required documents...your training completion certificate, a photo, the notarized  "Citizenship" form...any other explanations to other questions that may require such an explanation; your resume, three reference letters; a copy of your high school diploma, GED; college/university transcripts must be sent directly to us (do not upload those yourself).

- Take and pass the applicable State Standards Exam (RC or RC/AL) 

  -- The State Standards Exams are scheduled periodically (coordinated with the training sources to the extent possible) and you will need to "Register" and pay for the exam through our online "calendar" which is linked at the top of every page on our website.  There is a fee for the Exam  ($100) and a fee for licensure ($200)

  -- Passing the RC State Standards exam will complete the requirements for that licensure and only needs to be approved the Board at its next meeting (you should attend if able).

  -- If applying for RC/AL licensure, the applicant is eligible to take the NAB RC/AL exam after having passed the State Standards Exam.

     --- Once the RC/AL applicant has passed both the State and NAB exams, he is eligible to be licensed at the Board's next meeting (you should attend if able).


 

STEPS FOR ADULT DAY CARE  (ADC) APPLICANTS

- Take and pass the applicable State Standards Exam (ADC)

  -- Passing the ADC State Standards exam will complete the requirements for that licensure and only needs to be approved the Board at its next meeting (you should attend if able).
