CASE MANAGER, H21
BASIC PURPOSE: Positions in this job family are assigned responsibilities related to providing direct and indirect casework services to children and adults with developmental disabilities and their families.
TYPICAL FUNCTIONS: The functions within this job family will vary by level, but may include the following:
LEVEL DESCRIPTORS: This job family consists of four levels which are distinguished by the level of complexity and diversity of the specific job assignments, the extent or responsibility assigned for each level, the expertise required for the completion of duties assigned, and the responsibility assigned for providing leadership to others.
Level I Code: H21A Salary Band: I This is the basic level where employees are assigned responsibilities involving beginning level casework activities in a training status to build skills in providing case management services to consumers. In this role they will be assigned tasks which involve managing a caseload of moderate complexity and providing needed services; acting as the consumer’s QMRP, and monitoring all aspects of the consumer’s program and service delivery; and serving as the individual’s advocate. Additional responsibilities involve ensuring the development, implementation, monitoring/evaluation and modification of the Individual Habilitation Plan (IHP); including the development of individualized treatment plans; developing plans of care and reviewing the IHP/Plan of Care, notifying the consumer and the family/guardian/significant others with appropriate community resources; completing all necessary transition activities.
Knowledge, Skills and Abilities required at this level include knowledge of case management methods, principles and techniques; of types of developmental disabilities represented within the caseload; of types of providers and services available for clients; of interviewing methods, principles and techniques; of counseling methods, principles and techniques; and of adaptive communication techniques and nonverbal communication. Ability is required to manage a caseload of clients with developmental disabilities; to work cooperatively and effectively with other professionals in a team situation; to collect and analyze information; to make decisions relating to services provided to consumers; to develop a logical and practical individual habilitation plan for consumers with developmental disabilities; to evaluate the progress of consumers and the quality of their habilitation programs; to communicate effectively; and to mediate with providers and agencies to resolve problems.
Education and Experience requirements at this level consist of a bachelor’s degree in a human services field* and one (1) year of experience working directly with persons with developmental disabilities; or possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and one year of experience working directly with persons with developmental disabilities.*For purposes of the Case Manager job family “a bachelor’s degree in a human services field includes any degree from an accredited college or university except for a degree in a physical, natural or biological science or mathematics.
Level II Code: H21B Salary Band: J At this level employees are assigned responsibilities at the full performance level in managing a caseload of substantial size and complexity consisting of all disability areas; acting as advocate for the individual to ensure community presence and participation. Supporting the individual and family/guardian/significant others to make life decisions which lead to independence and interdependence; serving as the team leader of the Interdisciplinary Team (IDT); ensuring the development, implementation, monitoring/evaluation and modification of the IHP for desired outcome and developing the plan of care; including the development of individualized treatment plans. Additional responsibilities include coordinating guardianship procedures for adult consumers who do not have legal guardians; providing individual and family counseling to consumers and their families with social and/or emotional problems; assisting Level I case managers by furnishing information concerning case management practices, standards, rules and regulations and community resources, providers and programs; serving as resource individual for Level I case managers in providing information concerning specialized programs, services and treatments; and participating on committees at both local and state levels to formulate policies and procedures and to promote community awareness.
Knowledge, Skills and Abilities required at this level include knowledge of case management methods, principles and techniques; of types of developmental disabilities represented within the caseload; of types of providers and services available for consumers; of the behavioral sciences and allied disciplines involved in the evaluation, care and training of persons with developmental disabilities; of interviewing principles and techniques; of counseling principles and techniques; and of adaptive communication techniques and nonverbal communication. Ability is required to manage a caseload; to effectively intervene in crisis situations; to work cooperatively and effectively with other professionals in a team situation; to collect and analyze information; to make decisions relating to services provided to consumers; to develop a logical and practical plan of treatment for consumers with developmental disabilities; to evaluate the progress of consumers and the quality of their habilitation programs; to communicate effectively; and to mediate with providers and agencies to resolve problems.
Education and Experience requirements at this level consist of a bachelor’s degree in a human services field* and one year of professional experience working with persons with developmental disabilities in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field; or a bachelor’s degree in a human services field* and one year of professional experience in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field and one year experience working with persons with developmental disabilities; or possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and one year of professional nursing experience working directly with persons with developmental disabilities. *For purposes of the Case Manager job family “a bachelor’s degree in a human services field includes any degree from an accredited college or university except for a degree in a physical, natural or biological science or mathematics.
Level III Code: H21C Salary Band: K This is the specialist level where employees are designated as lead workers or back-up supervisors to Level I or Level II case managers. Responsibilities include assigning caseloads, providing training in caseload principles and practices and ensuring compliance with federal and state regulations and agencies policies, procedures and practices. Employees at this level will provide casework services at all levels of difficulty and complexity.
Knowledge, Skills and Abilities required at this level include knowledge of case management methods, principles and techniques; of laws policies, rules and regulations relating to the operation of the program; of types of developmental disabilities represented within the caseload; of types of providers and services available for consumers; of interviewing principles, methods and techniques; of counseling principles, methods and techniques; and of adaptive communication techniques and nonverbal communication. Ability is required to manage a complex caseload of consumers with developmental disabilities; to conduct group and individual training sessions; to intervene in crisis situations; to work cooperatively and effectively with other professionals in a team situation; to collect, analyze and present information; to make decisions relating to services provided to consumers; to develop a logical and practical individual habilitation plan of treatment for consumers with developmental disabilities; to evaluate the progress of consumers and the quality of their habilitation programs; to communicate effectively; and to mediate with providers and agencies in order to resolve problems.
Education and Experience requirements at this level consist of those identified in Level II plus two additional years of qualifying experience.
Level IV Code: H21D Salary Band: L This is the leadership level where employees are responsible for providing direct supervision to lower-level staff. Employees at this level are assigned direct supervisory responsibilities; will interview and hire case managers; administer in-service training programs for staff and professional providers and complete performance evaluations. Additional duties at this level include monitoring and reviewing casework of Level I or Level II case managers; participating in the ongoing development of division and program policies and procedures, including the implementation and evaluation of pilot programs; completing special reports; and communication policy changes to staff, and completing fair hearing/appeals review as per DHS policy and Office of Client Advocacy Investigations.
Knowledge, Skills and Abilities required at this level include knowledge of laws, policies, rules and regulations relating to the operation of programs for persons with developmental disabilities, including Title XIX, Medicaid, Federal and Oklahoma Social Security Acts; HMC/Homeward Bound Consent Decree, ICF-MR Regulations and agencies policies; of case management methods, principles and techniques; of planning and developing individual habilitation plans for persons with developmental disabilities; of types of providers and services available for consumers; and of supervisory principles and techniques. Ability is required to develop, coordinate and evaluate case management work; to conduct group and individual training sessions; to develop, coordinate and evaluate services for the developmentally disabled; to supervise the development of habilitation plans for consumers; to establish and maintain effective working relationships with others; and to communicate effectively, both orally and in writing.
Education and Experience requirements at this level consist of those identified in Level III plus one additional year of qualifying professional experience.
