Complaints Submission Page

Instructions for submitting a complaint

1-Print the complaint form by clicking on the button at the bottom of the page. 

2-Complete the complaint form with all required information

3-Include supporting documentation regarding the complaint (supporting documentation includes information such as a copy of a loan agreement, copies of any checks or indication of payments, correspondence to the lender or licensee, etc.)

4-Submit the complaint and supporting documentation via, fax, E-Mail or regular United States mail to:
