Complaints Submission

Instructions for submitting a complaint:

  1. Print the complaint form by clicking on the "Submit A Complaint" button below. 
  2. Complete the complaint form with all required information.
  3. Include all supporting documentation regarding the complaint. (Supporting documentation includes information such as a copy of a loan agreement, copies of any checks or indication of payments, correspondence to the lender or licensee, etc.)
  4. Submit the complaint and supporting documentation via, fax, E-Mail or regular United States mail to the address listed below.
