OHSO Grant Application Download Page (Please refresh browser to view latest update)

Thank you for your interest in submitting a grant application. The application form has been revised, so please take a moment to review these changes which are summarized below:

• All documents are available to view or download in Microsoft Word and Adobe PDF formats.  Documents in Corel WordPerfect format will no longer be provided.  You will need to have a version of Microsoft Word, Adobe Acrobat or the Adobe Reader (free download) to use these documents. To download the free Adobe Reader, click here (you may need to have administrative privileges on your computer to download and install the software).
• The instructions to complete the application is now a separate document and not included as part of the application itself. Please read the instructions and keep them handy as a reference while completing the application.
• The application form has been revised and is now a three page form. The requested information must be entered in the space provided, so brevity and optimum use of available space will be required. Do not submit additional pages. The Cost Assumption section has been removed and it is not necessary to provide as detailed of information in the budget section as in the past.
• The deadline for receipt of applications for the next fiscal year has been changed to March 31 of the current fiscal year (previously February 28).
