State Personal Financial Reporting Forms

The following forms must be used by state officers and state employees completing personal financial reporting

Deadline to file the form to cover Calendar Year 2012 is May 15, 2013

If your service ended anytime between the first day of this year and now, two forms are due from you. One form will disclose your financial interests for 2012, and the second form (your final) will disclose your financial interests for the time served during 2013.

• FORM F-1R (fill-in-the-blanks).  Statement of Financial Interest  -- for candidates, compensated state officers and Ethics Commissioners, and state officials (fill-in-the-blanks online, print, sign and date then fax or mail, or scan and email)
• FORM F-1R.  Statement of Financial Interest (for candidates, compensated state officers and Ethics Commissioners, and state officials -- print out paper form, fill in by hand, print, sign and date then fax or mail, or scan and email)
• FORM F-1S.  Supplement to Form F-1R
• FORM F-2R.  Statement of Financial Interests for Uncompensated Filers  (for public members -- members of boards and commissions including boards of regents)
• FORM F-3R.  Statement of No Change  (for candidates, state officers, board members, commission members and state officials.)
