State Personal Financial Reporting Forms

The following forms must be used by state officers and state employees completing personal financial reporting

Deadline to file the form to cover Calendar Year 2010 is May 16, 2011

If your service ended anytime between the first day of this year and now, two forms are due from you. One form will disclose your financial interests for 2010, and the second form (your final) will disclose your financial interests for the time served during 2011.

• FORM F-1R.  Statement of Financial Interests for Compensated Filers  (for candidates, compensated state officers and Ethics Commissioners, and state officials.)
• FORM F-1S.  Supplement to Form F-1R
• FORM F-2R.  Statement of Financial Interests for Uncompensated Filers  (for public members -- members of boards and commissions including boards of regents)
• FORM F-3R.  Statement of No Change  (for candidates, state officers, board members, commission members and state officials.)
