Forms for State Campaign Reporting
The Constitutional Ethics Rules govern state candidate and ballot measure campaigns. The following forms are available for viewing or printing. You may submit these forms on by facsimile transmission, hand carry or mail them to the Ethics Commission.
Previous forms are obsolete and will not fulfill the reporting requirements of the Rules.
- FORM SO-1. Statement of Organization (for candidate committees of candidates who file with the state election board.)
- FORM SO-2. Statement of Organization (for political action committees ["PACs"] to register.)
- STATEMENT OF INTENT (for candidate committees ONLY which do not intend to accept contributions or make expenditures in excess of $500 during the calendar year; must be filed by April 30 of that calendar year)
- AFFIDAVIT OF MINIMAL ACTIVITY. (for candidate committees ONLY which do not intend to exceed $500 in activity for the entire campaign.)
- CONTRIBUTOR STATEMENT (for candidate committees & PACs which receive contributions.)
- CONTRIBUTOR STATEMENT FOR FUNDRAISERS (for candidate committees and PACs.)
- FORM C-VR. Verification for Forms C-1R, C-3R and C-4R (for candidates ONLY to verify reports filed by their candidate committees.)
- FORM C-1R. Campaign Contributions and Expenditures Report (for candidate committees and PACs to report contributions and
expenditures during a reporting period.)
- SCHEDULES TO FORM C-1R. Schedules A-J to Form C-1R
- FORM C-3R. Statement of Inactivity (for candidate committees and PACs which have no activity during a reporting period.)
- FORM C-4R. Last Minute Contributions/Receipts Report (for candidate committees and PACs.)
- FORM C-5R. Independent Expenditure Report (for a person or non-candidate committees ONLY.)
- FORM C-6R. Electioneering Communications Report (for a person or non-candidate committees ONLY.)
