Behavioral Health Case Management Steps to Certification

 

Review the Case Management Administration Rules:

Please review the Chapter 50 Standards and Criteria for Certified Behavioral Health Case Managers that took effect July 1, 2011.  These will provide you with the qualifications, prerequisites needed and timelines for application and certification. Please review them closely as there have been critical changes.  

National Provider Identification Number:

  • It is extremely important that before you begin the On-Line Case Management Application process, you will need to get your National Provider Identification (NPI Number because that number will be your identification number in the Case Management System.  
  • If you already have an NPI number, please use that number.

Complete the Case Management On-Line Application:

  • Access to the new online system (click here) must create a user id and password first before you can access the new on-line application process.BE SURE TO KEEP YOUR USER NAME AND PASSWORD SOMEWHERE SAFE as you will need it each year to complete your renewal and anytime you need to update your information. If you cannot remember your user name and password, please contact the help desk at 405-522-0318.  DO NOT CREATE ANOTHER ACCOUNT AS THIS WILL DELAY PROCESSING OF YOUR APPLICATION.
  • New Case Management On-Line Application & Associated Forms – Once you have your user name and password, click on this link, choose the "Registration" icon.  Complete each tab with your information.  After you have completed each tab, enter the submit button and the Application Inventory page will appear.  Print this page and click on the link next to Employment Verification and then print this form.  Please make sure your information is complete and correct, but just incase you need to make corrections, revisions or updates, use this same link and make the changes yourself.
  • Once you have completed your on-line application, you only have six (6) months to complete the application process before your application expires.

Mail in the following documents:

  • Application Inventory – This has a list of all the needed attachments and should also be signed by the applicant.
  • Employment Verification Form with original signatures - The applicant completes the top portion of the form and their employer completes the bottom portion. Under Case Management Duties, please be sure to list the specific case management duties you will be performing.
  • Application Fee of $25.00** Check or money order only, made payable to ODMHSAS.
    **ODMHSAS employees are exempt from the initial fee of $25.00.
  • Official Transcript(s) – Only official transcripts will be accepted. Transcript must show degree and date conferred.
  • A copy of your official document showing that you are under supervision for licensure if applicable).  If you can provide a copy of your official under supervision agreement, you are not required to provide an official transcript.
  • A copy of your Professional License Card (if applicable) that shows the expiration date.  If you can provide with a copy of your Professional License Card, you are not required to provide an official transcript.
  • Mail all of the above to: ODMHSAS, Attention: CASE MANAGEMENT. The mailing address is P.O. Box 53277, Oklahoma City, OK 73152-3277.

Case Management Application Approval Process:

Applications will be reviewed in detail. Applicants who send in incomplete applications or applications needing corrections will be notified by e-mail of the needed corrections. These applications will be held for five business days to give the applicants time to make the corrections. If the corrections are not made within this window of time, the applications will be mailed to the applicants' attention at their noted place of employment.

Application Approvals:

After the Case Management Application has been approved, the applicant will receive an e-mail with an "Exam Approval Letter" attached.  Print the attached "Exam Approval Letter" and take it with you to the Exam site that you have selected.  Case Management exam sites are listed at the link below:

Schedule your Exam:

  • CareerTech Exam Site Information
  • Career Tech sites charge a $25.00 fee. 
  • Testing protocol requires an Oklahoma State issued identification, pre scheduling of exam, exam letter, etc., at all exam sites.

Successful Exam Completion

  • Case Manager Level I and the Case Manager Level II are not certified until they attend the Fundamentals of Case Management Training.  The training spots fill up very fast so don’t wait to fax (405) 522-8661 us your Coaching Report (exam results). Be sure to write the date of the training that you would like to attend on your Coaching Report.  Case Management training must be completed during the six month period that begins as soon as you submit your online application. Once the six month period has ended the Case Manger Level I and II that has not completed the training must wait a period of sixty (60) days before reapplying, and shall submit a new application.
  • Case Management III individuals will need to fax their Coaching Report to 405-522-8661 to indicate that they have completed the exam.  The candidates will be considered "Certified" as of the date of successful exam completion.  Case Management III individuals will receive their certificate within 30 days after we receive their Coaching Report. The Level III Case Managers will receive their certificates by email at the email address listed in the database.

Training Registration:

  • Once the applicant has successfully passed the exam, they will need to fax their Coaching Report (exam results) to 405-522-8661. Please see the Training Schedule.  Select a training you wish to attend and write the date of that training on your Coaching Report before you fax it.  You cannot enroll in training until we receive your coaching report.  Applicants will receive notification by e-mail once they have been enrolled in the training class.

Upon completion of the training, you will become a Certified Behavioral Health Case Manager
