Supervision: Licensed Professional Counselor
Both the Supervisor and the Supervisee are jointly responsible for insuring the requirements in Subchapter 11 of the LPC Regulations are fulfilled, the client’s rights to confidentiality are protected and the rules of their respective employers are adhered to during the course of supervision.

Failure to comply may result in the loss of supervised hours, denial of licensure, initiation of formal complaint procedures, and/or loss of approved supervisor status.
How to Begin:
In order to begin supervision you must meet the academic requirements as stated in Subchapter 9 of the LPC Regulations and you must submit an official application for licensure. An official application must include the following
- Application Form
- Internship/Practicum Documentation Form
- Three (3) Document of Recommendation Forms
- Application Fee of $145.00
- Official graduate transcript
- Completed criminal background check
In addition to the above mentioned requirements you must also submit a completed LPC Supervision Agreement, a completed On-Site Supervisor Verification Form and a completed Candidate Statement of Professional Disclosure.

Once the Department has received all of the above mentioned requirements/forms supervision may commence as of the approved date on the LPC Supervision Agreement. Any hours accrued prior to the approval date on the LPC Supervision Agreement will not be credited towards your supervised experience requirements and may result in the initiation of formal complaint procedures and/or loss of approved supervisor status.
How to Document Supervised Experience:
Documentation of supervised hours, evaluation of competence, date of observations, and dates of consultation between approved supervisor and on-site supervisor must be submitted on the LPC Evaluation of Supervised Experience Form. The evaluation form must be submitted on a semi-annual basis.
