Inactivate/Retire:
You may place your license on inactive status, if your license is in good standing, by submitting a request to be placed on inactive status, a one-time $25.00 fee, and your original license. A license placed on inactive status must remain inactive for at least one (1) year, but may remain inactive for as long as you desire. You do not need to accrue continuing education or pay any additional fees while your license is inactive. You may reactive your license by submitting a reactivation request form and the prorated fee for the month you wish to reactive. Additional information regarding reactivation requirements is on the reactivation request form.

If your license is in good standing, you may retire your license by notifying the Department in writing. A retired license cannot be reactivated but it does not prevent an individual from reapplying for licensure in the future.
Late Renewal:
If you fail to renew your license by the expiration date, your license will be suspended forfeiting all right and privileges granted by the license. You have the right to renew the license by payment of the $100.00 renewal fee, the $25.00 late renewal fee and submission of a completed continuing education roster documenting the prorated hours of NAMP approved continuing education.
Licensees not renewed within the one (1) year renewal period shall be permanently expired and shall not be reinstated. The license shall be returned to the Department.
