Inactivate/Retire:

You are not required to accrue continuing education or pay any additional fees while your license is inactive. You may reactivate your license by submitting a Reactivation Request Form and the prorated fee for the month you wish to reactivate. Additional information regarding reactivation requirements is on the Reactivation Request Form.

If your license is in good standing, you may retire your license by notifying the Department in writing. A retired license cannot be reinstated but it does not prevent an individual from applying for a new license in the future.

Late Renewal:
If you fail to renew your license by the expiration date, your license will be suspended, forfeiting all rights and privileges granted by the license. You have the right to reinstate the license by payment of the $100.00 renewal fee, the $25.00 late renewal fee and submission of a completed continuing education roster documenting ten (10) hours of NAMP approved continuing education.

Licenses not renewed within the one (1) year renewal period shall be permanently expired and shall not be reinstated. The license shall be returned to the Department.

Frequently Asked Questions
Q: How long will it take to receive my new verification cards?
A: The PCL Division processes renewal materials on a daily basis. However, the Oklahoma State Department of Health experiences an increase in the amount of mail received during the month of December. This influx creates delays in the processing of our mail. In order to avoid delays, please submit your renewal materials as early as possible.
