Inactivate Status/Reactivation and Retirement:

Inactivate Status:
You may place your license on inactive status, if your license is in good standing, by submitting a request to be placed on inactive status, a one-time $25.00 fee, and your original license. A license placed on inactive status must remain inactive for at least one (1) year, but may remain inactive for as long as you desire. You do not need to accrue continuing education or pay any additional fees while your license is inactive.

Reactivation:
You may reactive your license by submitting a reactivation request form and the prorated fee for the month you wish to reactive. Additional information regarding reactivation requirements is on the reactivation request form.

Retirement:
If your license is in good standing, you may retire your license by notifying the Department in writing. Although a retired license cannot be reactivated, it does not prevent an individual from reapplying for licensure in the future.
