Duplicate Cards/Certificate Request:

In order to receive a duplicate card(s) or certificate(s) you must complete and submit the Duplicate Card/Certificate Request Form. There is not an administrative fee for duplicate cards. However, there is a $25.00 fee for a duplicate certificate.

Please note: In order to change your degree you must submit an official transcript documenting the change.
