Home Services Division

Home Services Division (HSD) staff strive to ensure compliance with minimum standards and the provision of quality care.  HSD staff perform on-site surveys to ensure compliance with standards, issue licenses, monitor compliance, conduct home visits to clients receiving services, investigate complaints and sanction facilities that fail to comply.  Every person, corporation, partnership, association or other legal entity desiring to obtain a license to establish, or to obtain a renewal license to operate a home health agency in this state must make application to the Department in such form and accompanied by such information as the State Commissioner of Health prescribes.

Home Health Agencies
Oklahoma licensure of home care agencies is required by Oklahoma Statute Title 63, Section 1-1960 et seq. Oklahoma Administrative Code Title 310; Chapter 662

Hospice Agencies
Oklahoma licensure of hospice agencies is required by Oklahoma Statute Title 63, Section 1-860, 1 et seq,; Oklahoma Administrative Code Title 310: Chapter 661
