Complaints & Certification Review

Medical Facilities Service receives and investigates complaints against non-long term care facilities for non-compliance with Medicare regulations and/or State licensure requirements. Complaints against non-long term care facilities may be filed electronically through the Medical Facilities Complaint form or by calling 405-271-6576 in the Oklahoma City area or 1-800-234-7258 state-wide, or by written statements provided to the Division.
