Monthly Quality of Care Reports

Each facility’s individual Quality of Care Report is available for public viewing at the Oklahoma State Department of Health, Tenth Floor, 1000 NE 10th Street, Oklahoma City. The Oklahoma State Department of Health's hours of operation are 8:00 a.m. to 5:00 p.m., Monday through Friday, state holidays excepted. 

Copies of the monthly Quality of Care Reports may be made upon request to the Oklahoma State Department of Health and upon payment of reproduction charges. The cost will be about $25 to $30 for each month of reports for all 425 facilities. More than 300 facilities report electronically -- those reports are available in Excel spreadsheets on one CD-ROM, for the cost of $10.00. Some facilities file submit reports on paper -- cost for hard copies is $0.25 per page. If the report request is for select facility or facilities, instead of all facilities per month, a research fee of $25.00 per hour will apply in addition to the reproduction charges.

Occupancy Reports

We are pleased to resume providing nursing home occupancy data. Beginning in June 2004, occupancy data for the review of Nursing Facility Certificate of Need applications must be based on monthly reports that are filed with the Oklahoma Health Care Authority pursuant to Title 63 O.S. Section 1-1925.2(H). Each nursing facility files a monthly "Quality of Care Report" that includes detailed shift-by-shift counts of residents in the facility. The data from those reports are compiled into the following reports. Any person interested in pursuing certificate of need approval for a bed expansion or a new nursing facility may rely on the occupancy statistics below. Reports for each month are sorted by facility type, then county.

Historical Reports

Occupancy reports prepared by the Oklahoma State Department of Health for the period from to January 2001 to June 2004 are available through the links below. These reports were based on monthly reports submitted by nursing facilities to the Oklahoma State Department of Health. Prior versions of these reports are available through the Publications Clearinghouse of the Oklahoma Department of Libraries or from office archives on request. These types of reports are not available after May 2004.

 *HB 2723 that was signed and effective the week of June 7, 2004 eliminated the traditional Nursing Home Occupancy Report therefore the May 2004 report is the last report of this type for nursing facilities.
